Please fill out a New Client Form in advance of your first appointment and either email it to us at office@goodfriendsanimal.com, fax it to 217-344-0654, or bring it with you to your appointment.

Some services, like surgery and dental cleanings, require that we have a consent form on file. You can download the form here and either print and fill it out, or complete it within Adobe Reader and then print it. Either way, please bring the completed and signed form to your pet's appointment.
